Back-to-school physicals, dental exams, immunizations & sports physicals

Important information for families with new students (including our incoming kindergarten students) and those entering grades 3, 6, 7 and 11 as well as our Fort Health Couch and USC High School student-athletes. Please make sure your child’s health information is up-to-date.


Physical Exams: Kindergarten, Grade 6 and Grade 11

The Pennsylvania School Code mandates medical examinations for students entering kindergarten, grade 6 and grade 11 because these grades represent important periods of growth and development in a student’s life. The major objective of these examinations is to ensure that the student is in a general state of good health.

You can download a Private Physician Physical Form for your child's pediatrician to complete or use their electronic physical form. Examinations completed on or after Sept. 1, 2023 (one year prior to the current school year), may be used to fulfill the requirement as we do accept physicals from the previous year due to insurance reasons.

If your family does not have an established pediatrician, your child will be given an opportunity (with signed parental permission) to be examined by our school physician in the spring of 2025.  

We understand if you have concerns/hardships getting into your pediatrician at this time. The Upper St. Clair School District will accept physicals throughout the 2024/2025 school year to complete the requirement.

Dental Exams: Kindergarten, Grade 3 and Grade 7

The Pennsylvania School Code mandates dental examinations for students entering kindergarten, grade 3 and grade 7. This mandate can be completed by a private dentist or the school dentist. You can download a Private Dentist Examination Form for your child's dentist to complete or use their electronic form. Examinations completed on or after Sept. 1, 2023 (one year prior to the current school year), may be used to fulfill the requirement as we do accept dental exams from the previous year due to insurance reasons.

If you are unable to make an appointment with a private dentist, we would be happy to schedule your child to be examined by our school dentist. The school dental examinations will take place in the spring of 2025 with parent permission. 

If you have any questions or concerns, please do not hesitate to contact your school nurse.

We understand if you have concerns/hardships getting into your dentist at this time. The Upper St. Clair School District will accept dental exam forms throughout the 2024/2025 school year to complete this requirement.  


Turning in completed forms

If you have completed dental and/or physical forms to turn in, you can do any of the following:
  • Have your child turn in the form(s) in-person when school begins
  • Mail the completed forms to the school. Please include "Attn. School Nurse" in the address line.
  • Scan and email your completed forms directly to your child's school nurse. click here for our health services staff by school
 

2024-25 School Vaccinations

The school nurse maintains an official Pennsylvania immunization record for each student. When your child receives additional immunizations, please send information to the school nurse to ensure that your child’s record is up-to-date.

In accordance with the Allegheny County Health Department, the immunization guidelines for students are provided below.

All Grades K-12

  • 4 doses of tetanus, diphtheria, and acellular pertussis* (1 dose on or after the 4th birthday)
  • 4 doses of polio (4th dose on or after 4th birthday and at least 6 months after previous dose given)**
  • 2 doses of measles, mumps, rubella***
  • 3 doses of hepatitis B
  • 2 doses of varicella (chickenpox) or evidence of immunity  
For attendance in 7th Grade
  • 1 dose of tetanus, diphtheria, acellular pertussis (Tdap) on the first day of 7th grade.
  • 1 dose of meningococcal conjugate vaccine (MCV) on the first day of 7th grade.
For attendance in 12th Grade
  • 1 dose of MCV on the first day of 12th grade. If one dose was given at 16 years of age or older, that shall count as the 12th grade dose.

Sports Physicals/Online Athletics Registration

In order to participate in fall sports at Fort Couch Middle School or Upper St. Clair High School, student-athletes must submit their completed sports physical forms prior to the start of the first fall practice. Sports physicals must be completed May 1, 2024, or after and are valid for one year.
