Required Documentation
As a condition of employment, ALL candidates must provide documentation of the following:
- School Personnel Health Record or Physical, must have been completed within 12 months of the hire date
- Tuberculin or PPD Test, must be within three months of the hire date (the documentation is included within the physical form)
- Drug Screen Test/Work Capacity Evaluation, must be completed by St. Clair Occupational Medicine (form is available from human resources)
- Work Capacity Evaluation, only applies to nutrition center and custodial/maintenance staff
- Work Capacity Evaluation, only applies to nutrition center and custodial/maintenance staff
- Act 24 (PDE 6004 Form - Arrest/Conviction Report)
- Act 168 (Sexual Misconduct/Abuse Disclosure Release)
