Mandatory Dental Exam for Students in grades K, 3, and 7
The Pennsylvania school code mandates dental examinations for students in grades K,3 and 7. This is to ensure that all students are being evaluated for oral and dental issues during these pivotal ages of growth.
This requirement may be met in two ways, a physical done by the student’s private dentist or by the school dentist. If a private dentist is preferred, please have your dentist compete and return the Private Dental Examination Report form.
Those students who do not return a private dentist form will be scheduled to see the school dentist in the spring with parental permission. If you have trouble viewing the form please contact your school nurse or visit your school secretary during summer hours to pick up a blank form.
